Health Office

Message from our Nursing Staff: If your child is sick, PLEASE keep him/her home. Call the attendance office at 732-739-0800 x 124 to report the absence. If you take your child to the doctor, please ask for a note and bring it in when returning to school.

General Information

All new students are required to have a physical by their private physician. No student will be admitted unless physical forms and immunizations are complete and up-to-date.

    Any student who uses an inhaler, must have this form completed, signed, and returned to the Nurse. Please follow the directions on Page Two to complete Page One of the Form. Make sure that there are signatures of a parent and a physician as well as the physician's stamp on the completed form.
