Hall of Fame Nomination Form

The Saint John Vianney Athletic Hall of Fame was established in 1994 to recognize the achievements of our Alumni and Coaches who have excelled in their athletic careers during and after their time at SJV.

To be inducted into the Hall of Fame nominees must meet specific criteria, which may include:

Being a State Champion while at SJV, being recognized on an All-State team while at SJV, being an outstanding scholar-athlete who has brought honor and fame to SJV by their high school academic and/or professional career, and Coaches who have made incomparable contributions to the success of an SJV athletic program.
All nominees, who are alum, must have graduated at least 5 years prior to nomination.
All nominations must be received by October 15th
